THREE PEOPLE did not have to die in a 2005 gas explosion at a Bergenfield apartment building.

The explosion, the deaths and the injuries could all have been prevented except for a series of mistakes by a tank-removal company, a gas utility and the Bergenfield fire department. That is the sad conclusion of a federal investigation of the December 2005 blast that tore off nearly half of a 24-unit brick building and spewed shattered glass more than half a mile.
